In this position, you will act as a virtual care coach helping people with chronic conditions. Care Coaches help onboard patients, deliver RPM and/or CCM programs, review and triage remote monitoring alerts, collaborate with an extended care team, coordinate care, follow and update patient care plans, and support patients in achieving their health goals.. Important Note on Daily Volume:. You should expect to spend most of your day on the phone and computer, including making approximately 30–70 outbound calls per day. This role is best suited for candidates who are highly comfortable in a fast-paced, high-volume, phone-based outreach environment.. Primary Duties and Responsibilities:. Manage a panel of patients (up to 225), providing monthly care and support via phone and electronic communication under physician supervision..
