We are seeking warm-hearted, board-certified physicians (MD/DOs) to partner with IMPaCT Care as Physician Partners to play a short-term, high-impact role, helping identify health conditions and care gaps, stabilizing medical concerns, and connecting patients to longer-term primary care solutions.. . Dedicate 5–10 hours per week to real-time telehealth visits with patients referred by CHWs including consults, prescribing medications when appropriate, and providing clinical guidance..
