Ennoble Care is seeking a self-motivated, goal-oriented Social Work Care Coordinator to support the growth and expansion of our Behavioral Health Integration (BHI) and GUIDE dementia care program. This full-time, fully remote position is ideal for a dynamic individual with a passion for improving patient outcomes through proactive outreach, referral development, and care coordination..  . . This dual-role coordinator will play a critical part in driving enrollment, educating patients and families, and supporting care teams through high-impact, multitasking responsibilities across both programs. The ideal candidate thrives in a fast-paced, evolving environment and is eager to grow alongside innovative healthcare initiatives..  . . Responsibilities:.  . . . Conduct eligibility screenings for the BHI and GUIDE programs.  . . . . Perform outreach (phone, email, and mail) to patients and caregivers who are hard to reach or newly referred.  . . . . Educate patients and families on the purpose and benefits of BHI and GUIDE services.  . . . . Participate in cold call and warm outreach campaigns to increase program engagement.  . . . . Assist with marketing and referral goals for both programs, including tracking outcomes.  . . . . Serve as the first point of contact for incoming questions and calls related to specialty care programs.  . . . . Collaborate with the interdisciplinary team (IDT), including primary care providers and care managers, to ensure aligned and timely communication.  . . . . Help coordinate resources, appointments, and care plan updates as needed.  . . . . Participate in team meetings and program growth strategy sessions.  . . . . Support occasional administrative tasks and outreach projects (e.g., mailings, spreadsheet tracking).  . . .  . . Qualifications:. . . High school diploma or equivalent required (bachelor’s degree in social work, Psychology, Public Health, or related field strongly preferred).  . . . . Background or strong interest in social work, behavioral health, dementia care, or care coordination.  . . . . Self-starter with strong time management and multitasking abilities.  . . . . Excellent communication skills – verbal, written, and interpersonal.  . . . . Proficient in Teams, Microsoft Excel, Word, OneDrive, and remote communication tools.  . . . . Comfortable with goal tracking, outreach targets, and evolving program workflows.  . . . . Willingness to learn about available social service and community-based resources.  . . .  . .
