We're looking for a motivated, detail-oriented Clinical Coordinator to join our San Francisco office and help us continue delivering exceptional care to the patients and families who depend on us.. . ABOUT THE ROLE. . This is a fast-paced, high-impact position at the heart of our clinical operations. As a Clinical Coordinator, you'll serve as the connective tissue between our field clinicians, patients, families, and physicians — keeping care plans on track, documentation accurate, and communication seamless. Coordinate patient care and provide essential administrative support to field clinicians. . Track, clarify, and process medical orders, ensuring complete and accurate documentation. . Act as the primary liaison between patients, families, and physicians regarding care plans and updates. . Input orders and upload documentation into the electronic medical records system. . Assist with the onboarding and orientation of new clinical staff. . Order and monitor medical supplies to maintain inventory levels and ensure OSHA compliance. . Conduct chart audits to ensure accuracy, completeness, and regulatory compliance. . Support overall office operations and take on additional responsibilities as needed. . . WHAT WE'RE LOOKING FOR. . . Current RN license in California (required). . 1+ year of nursing experience; home health experience strongly preferred. . Associate degree or higher preferred. . BLS certification is a plus. . Exceptional verbal and written communication skills with the ability to manage high call volumes professionally. . Strong organizational and multitasking skills — you can juggle priorities without dropping the ball. . Comfortable with Apple products and proficient in medical scheduling and documentation software. . A service-first mindset with genuine warmth toward patients, families, and colleagues. . . SCHEDULE & LOCATION. . . Monday – Friday, Day Shift. . On-site at our San Francisco office (remote work not available for this role). . . WHAT WE OFFER. . .
