Community Care Specialist at New Day Admin. New Day Healthcare is currently seeking a Community Care Specialist. Do you have a passion for making a difference in someone’s life? Receive incoming calls and messages for potential clients and consistently strive for emotional linkage and developing relationships . Determine client needs and assist in coordinating care by referring to appropriate service lines . Collect, share, and manage information using respective databases . Clearly communicate detailed information to appropriate teams . Track referrals, keep status and notes up-to-date in database, and advocate for patient care . Internally build relationships for the furthering of serving clients . Make outgoing calls to patients in the care continuum . Performs other related duties as directed by Supervisor .  .  . Qualifications, skills and abilities .  . Patient, attentive, and empathic listener . Must have a strong phone etiquette with the ability to communicate well; both verbally and in writing . Must be detailed-oriented, accurate, organized, multi-tasker, and great with time-management . Initiative-driven individual able to work independently and in a group setting  in a fast-paced environment . Computer proficiency and knowledge of Microsoft products . Must be reliable, self-starter, strong work ethic and work well within a team . Previous sales or marketing preferred .  Experience in healthcare preferred . Will train the right candidate .  .
