
Outpatient Case Manager at Remote Raven. Position Summary. The Outpatient Case Manager provides remote, client-centered support across all stages of treatment, including intake, active care coordination, appointment scheduling, and discharge planning. This role ensures the efficient delivery and documentation of wraparound services by coordinating with clients, families, internal teams, and external service providers. The Case Manager works closely with clinical and operations staff to track client progress, resolve barriers to care, and support treatment goals in accordance with the individualized treatment plan.. Essential Duties and Responsibilities. . Serve as the primary point of contact for assigned outpatient clients and their families to support non-clinical needs and enhance treatment outcomes.. . . Coordinate all in-person and virtual therapy and psychiatric appointments. , including proactive communication with providers to determine frequency of services and with clients to confirm attendance.. . Document all appointments and ensure accurate calendar management across clinical schedules.. . Track and report no-shows, cancelations, and reschedules; notify Finance of applicable no-show charges for billing.. . Monitor client participation in scheduled services and follow up on attendance, engagement, and any missed appointments.. . Coordinate client intakes and discharges, ensuring accurate and timely completion of all required documentation.. . Identify barriers and needs across housing, education, transportation, healthcare, legal services, and other social determinants of health, and provide linkage to appropriate resources.. . Maintain ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.. . Actively participate in interdisciplinary team meetings to review and support treatment plan goals.. . Escalate any urgent or critical issues (e.g., safety concerns, sudden disengagement, or emergent needs) to the Clinical Director immediately.. . Uphold professional ethics and confidentiality standards in all communications and records.. . Utilize electronic systems (EMR, CRM, communication platforms) to track and report all client-related activities.. . Perform other case management duties as assigned by leadership.. . Qualifications. . 2+ years of experience in behavioral health, social services, or case management. . Strong written and verbal communication skills. . Proficiency in Google Workspace and/or Microsoft Office. . Reliable high-speed internet and ability to work from a secure remote location. . Ability to obtain a Level One Fingerprint Clearance Card. . Preferred. :. . Associate’s or Bachelor's degree in Social Work, Psychology, or related field. . Experience working with adolescents and families. . Knowledge of Medicaid, state behavioral health systems, and community-based services in Arizona. . This is a 100% Remote job. Full time.
