Accountabilities. . Conduct initial intake calls and ongoing patient outreach to review and update care plans, monitor progress, and address emerging needs.. . Coordinate care for a panel of complex patients, collaborating with providers, social workers, behavioral health specialists, and clinical pharmacy teams.. . Support patients during care transitions, including post-hospitalization follow-ups, discharge planning, and medication reconciliation.. . Implement interventions to reduce inpatient admissions, readmissions, emergency room visits, and support patients in remaining safely in their homes.. . Participate in interdisciplinary team huddles and quality improvement initiatives.. . Mentor new hires and existing case management team members.. . Adhere to compliance policies, procedures, and applicable laws and regulations.. . . Current RN license in good standing in the state of practice.. . Bachelor’s degree in nursing required, or an associate degree in nursing with a relevant clinical or business bachelor’s degree.. . Minimum of 4 years of clinical experience, with at least 2 years in case management in home health, ambulatory care, community public health, or insurance settings.. . Certified Case Manager (CCM) certification preferred or commitment to complete when eligible.. . Experience with geriatric care, discharge planning, utilization management, and managed care preferred.. . Strong knowledge of Medicare and Medicaid regulations, clinical best practices, and evidence-based care.. . Excellent communication, organizational, and critical thinking skills, with the ability to manage multiple complex patients.. . Proficiency in Microsoft Office Suite and clinical EMR systems.. .
