Deliver expert virtual nursing care to a diverse adult population across hospital, skilled nursing, and in-home settings.. . Partner closely with care teams to assess, plan, coordinate, and evaluate patient care.. . Provide patient and family education and serve as a mentor and coach to fellow nurses.. . Supervise and direct support staff to ensure compliance with evidence-based practices.. . Monitor patient status using advanced telemetry and clinical tools.. . Identify risks and define appropriate interventions using critical thinking and clinical expertise.. . Maintain accurate documentation using various EHR systems (Epic, Cerner, Meditech, etc.).. . Collaborate with interdisciplinary remote teams and originating sites for care coordination.. . . At least 5 years of recent experience in acute, critical care, or step-down settings.. .
