Accountabilities:. . Assess, identify, and secure services tailored to participants’ unique needs while monitoring progress.. . Support the Project Manager in developing and executing new programs and initiatives.. . Respond promptly to participant requests and facilitate access to adaptations, medical equipment, transportation, and other resources.. . Ensure compliance with all documentation, assessments, service plans, and reporting requirements within established timeframes.. . Maintain detailed, person-centered records of visits and contacts using required systems.. . Consistently demonstrate organizational values in all professional interactions and duties.. . . Bachelor’s degree required; fields such as sociology, social welfare, psychology, gerontology, or criminal justice preferred.. . Minimum three years of case management experience, preferably within the PA Office of Long Term Living.. . Strong communication, customer focus, integrity, and problem-solving skills.. . Proficiency with relevant computer systems and technology.. . Physical ability to perform duties including reaching, stooping, walking, standing, and lifting up to 20 pounds.. .
