Perform Start of Care (SOC) admissions and complete OASIS documentation for new patients.. . Conduct follow-ups and evaluation visits on weekends in the assigned territory.. . Monitor and document patient conditions and coordinate care plans with physicians and internal teams.. . Administer IV medications such as antibiotics and diuretics in accordance with protocols.. . Provide hands-on patient care including wound treatment and medication reconciliation.. . Maintain accurate and timely documentation of all care interventions and outcomes.. . Collaborate with an interdisciplinary team to ensure the highest standards of care delivery.. . . Active and unrestricted Registered Nurse license in California.. . Current BLS/CPR certification and valid CA driver’s license.. . Experience with Start of Care (SOC) and OASIS documentation preferred.. .
