We’re looking for a Patient Care Coordinator to support end-to-end scheduling and care coordination for patients receiving both telehealth and in-person services across multiple U.S. states. This role involves managing high-volume scheduling, coordinating referrals with external providers, and communicating clearly with patients, physicians, and facilities to ensure care is delivered smoothly and on time.. Scope of Work/Responsibilities. 1) End-to-End Patient Scheduling & Care Coordination. Manage the full scheduling lifecycle for patients requiring telehealth and in-person care. Schedule telehealth visits with internal physicians across 20+ medical specialties. Coordinate in-person services such as MRI, X-ray, laboratory testing, and specialty clinic visits. Handle urgent and same-day scheduling based on clinical priority and physician direction. Support patients across multiple U.S. states and ensure proper regional coordination. Assess and sequence appointments correctly to prevent delays or incorrect bookings. 2) Referral Management & External Provider Coordination. Coordinate with external imaging centers, clinics, and specialty providers. Confirm appointment availability, preparation instructions, prescreening needs, and required documentation. Identify and resolve common referral barriers, including: Insurance authorization issues, Missing clinical records, & Incomplete referrals/documentation. Follow up consistently until: Services are completed & Reports/results are received. Maintain continuity of care across multiple providers and healthcare systems. 3) High-Volume, High-Stakes Communication. Manage frequent inbound/outbound communication with: Patients, Physicians, Imaging centers, & Specialty clinics. Explain appointment details, prep instructions, and next steps clearly to patients. Handle urgent, sensitive, or high-stress situations with professionalism and empathy. Collaborate closely with internal schedulers and physicians to ensure seamless coordination. Communicate effectively with Spanish-speaking patients and provider offices (preferred). Experience requirements. Medical education/degree required: Not required. Previous experience as a virtual assistant required: Preferred (healthcare scheduling/care coordination experience is a plus). Medical license (doctor, nurse, PT, etc.) required: Not required. Basic requirements. Must be proficient in speaking and writing English very clearly. Must have relevant work experience. Be able to submit an NBI clearance and/or Local Police Clearance background check before onboarding [mandatory]. Must be available for video meetings with your camera on (when needed).
