The CHI Specialist supports patients with identified social determinants of health (SDOH) needs through outreach, resource navigation, and care coordination under the general supervision of the billing practitioner. Work is non-clinical unless appropriately licensed.. . . Key Responsibilities:. Patient Outreach & Engagement. Conduct telephonic or digital outreach to assigned patients. Perform standardized SDOH screenings. Identify barriers such as food, housing, transportation, and utilities. Engage patients using approved engagement techniques. Resource Navigation. Connect patients to appropriate community-based resources. Facilitate referrals and warm handoffs. Assist with applications for social services when appropriate. Track closed-loop referral completion. Care Plan Support. Reinforce practitioner-directed care plans. Document patient goals and progress. Escalate clinical or safety concerns to RN or practitioner. Maintain accurate monthly time logs. Documentation & Compliance. Record all encounters in the designated platform/EHR. Maintain HIPAA compliance at all times. Follow incident-to and CHI program requirements. Participate in QA reviews and required orientation updates. Qualifications:. Minimum:. High school diploma or equivalent. Strong phone-based communication skills. Ability to work independently in a remote environment. Physically located in the United States. HIPAA-compliant home workspace. Required prior to independent case work:. Must successfully complete CircleLink CHI orientation and competency validation before independently performing billable CHI services. Must adhere to all company protocols, scripts, and documentation standards. Preferred Qualifications (. Not Required. ). Experience in care coordination, community health, social services, or patient navigation. Community Health Worker (CHW) certification. Medical Assistant, LPN, RN, or Social Work background. Experience with Medicare care management programs. Bilingual (Spanish strongly preferred). Experience working with high-risk or Medicare populations. Technical Requirements (Remote Work). Private, secure home workspace. Reliable high-speed internet. Computer capable of running required systems. Headset for patient calls. Ability to access systems via secure login/VPN. Compliance with all HIPAA safeguards. Supervision & Scope. Work performed under general supervision of the billing practitioner. Must escalate any clinical or safety concerns immediately. May not provide clinical advice unless appropriately licensed and authorized. Must complete required orientation and maintain competency status. . Productivity Expectations. Meet monthly case completion targets. Maintain quality and documentation standards. Achieve required patient engagement metrics. Participate in periodic orientation updates and QA review.
