Accountabilities:. . Conduct telephonic outreach to enroll patients into care programs and build trusting relationships as their main point of contact.. . Collaborate with Nurse Care Managers to develop and implement personalized care plans tailored to patient needs.. . Coordinate care transitions across healthcare settings and dialysis centers, ensuring continuity and strong social support.. . Assess and address Social Determinants of Health—connecting patients to vital services such as housing, transportation, and food support.. . Navigate patients through health systems and community-based resources, removing barriers to care.. . Advocate for patients, identifying and addressing inequities and ensuring timely interventions.. . Maintain proactive, clear communication with patients, caregivers, and clinical teams to ensure aligned care delivery.. . Minimum of 3 years of experience in healthcare coordination, community health, or a related field.. . Proven success working in a collaborative, team-based care model.. .
