Navigate multiple electronic referral software portals. . Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships. . Communicate effectively with patients and patient care givers. . Qualifications for Intake Associate - Per Diem - Remote:. . Must reside in New Hampshire. . 2 years of Health Care experience. . Knowledge of medical terminology, familiarity with a medical office setting, and/or triage. . Knowledge of Home Health/Hospice. . Must be comfortable working in a remote environment. . Proof of Flu vaccination. . Reliable transportation. . Valid driver's license. . Automobile insurance. . Physical exam within the last year. . Two-step TB test. .
